Information concerning the Vermont Ski Trip for this winter season
Complete, Sign and Turn In the Health History/medical form and if needed the Self-Administering Medical Release form. Turn the health form in too Mr. Temple in the athletic office.
Steps to register a high school student for the trip:
In order to be registered for a trip, a student must complete ALL THREE of the following steps. Failure to complete all of the three steps will result in the student not being added to the list and they miss out since the trip sign-up is on a first come first serve basis.
1. PARENTS must digitally complete and digitally sign the
Vermont Registration and Waiver form
Students are not to fill out the registration form, but they should be aware of the school rules and policies detailed in the waiver form.
2. Health Form

Download, Print, Complete, sign and turn in the Self-Administering Medical Release form – only if needed
please mark your envelope with the student’s name and “ski club” so that the office doesn’t have to open the envelopes.
3. PAYMENT: Turn in cash or check (payable to) “Lewisburg Area School District”
- Provide a deposit or full payment – TOTAL COST OF THE TRIP: $325.00
Deposit = $100.00 by December 8th
2nd Payment = $225 by January 5th
Families with multiple children going on the trip may combine payments into one check. Students may pay all at once, or with two payments.

Sign-ups will be accepted on a first come, first-served basis due to the limited number of seats on the bus. If enough students sign up we may increase to two buses. However, a waiting list will be established until the status of the second bus is determined.
Fees are non-refundable. Students must notify the Ski Club Vermont Advisor prior to the scheduled ski trip if they will be unable to attend. Deposits will not be returned unless the advisor can secure another student to take the open seat.
Trip Itinerary and price inclusions: Leave LHS at 2:00 pm on the 2nd, return at 11pm on the 4th
- Round-trip motor coach to and from Vermont
- Two (2) nights accommodations at the Holiday Inn Express, Rutland Vt.
- Two Continental Breakfasts
- One Dinner
- Two day, all mountain lift tickets to Killington
Rentals are available for students without their own equipment at an additional cost.
******** Students are strongly encouraged to wear helmets.

In order to chaperone adults must complete the following clearances, submit them to Julie Witmer in the district office and get school board approval as a volunteer.
Clearances required are:
Act 34 Clearance – PA State Police Criminal History
Act 151 Clearance – Child Abuse
Act 114 Clearance – FBI National Criminal History or Affidavit
Mantoux TB Test
Mandated Reporter Training
